Course fee*	1000 CHF per delegate
Payment - On receipt of your online registration an invoice will be emailed to you for payment of the course fee. All invoices are in Swiss Francs and payable within 30 days and/or prior to the course dates.
Please give your company VAT number in full in the box below - this will appear on the invoice. Invoices issued to Swiss addresses will be subject to 7.6% Swiss VAT. All other invoices will not carry VAT.

Declaration - Please check your online booking carefully before emailing to EphMRA because bookings cannot be changed or cancelled once submitted.
Cancellations for registrations must be sent to generalsecretary@ephmra.org by email and not by phone. Cancellation - before 15 September 2010 - full refund less a cancellation fee of CHF 150 per person. After 15 September 2010 - no refund at all. EphMRA assumes that individuals or companies will have insurance cover for illness etc. Name substitutions can be made at any time. If you cancel after 15 September 2010 but your fees have not been paid then you are still liable for payment.
